Clinicopathologic Evaluations of 500 Cellular Fibroadenoma, Benign and Borderline Phyllodes Tumors Indicate the Current Classification May Not Have Clinical implications
Background: The diagnostic criteria and clinical implication of
benign fibroepithelial lesion classification are not clear.
Design: This study included 500 cases including 65 cellular
fibroadenoma (cFA), 202 benign (BePT) and 165 borderline (BoPT).
BePT was diagnosed when all 5 morphologic features were present (well defined border, mildly/moderately increased stromal
cellularity, mild/moderate stromal atypia, <5mitoses/10HPFs,
absence of stromal overgrowth). BoPT was diagnosed when the
tumor lacked at least one of the 5 morphologic features but did
not meet our newly proposed diagnostic criteria for malignant
phyllodes tumor (PMID: 38934254). cFA was diagnosed when no
leaf-like structure was seen. Variables including age, race, tumor
size, leaf-like structure, stromal cellularity and atypia, mitosis,
tumor border, stromal overgrowth, stromal heterogeneity, stromal
myoid change, stromal necrosis, satellite nodule, epithelial proliferation and surgical margin status were evaluated. The median
follow-up time was 82.5 months (0.03 to 263 months).
